Lead clinical care, supervise STNAs and LPNs, and coordinate with physicians and family caregivers. Ohio RN license required.
Deliver direct nursing care under RN supervision — medication administration, vitals, treatments, documentation, and family education.
Provide hands-on personal care under the direction of nursing staff — ADLs, vitals, hygiene, mobility, and observation.
Support adults and youth with disabilities at home and in the community — personal care, companionship, skill-building, and community engagement.
Provide reliable non-medical transportation to work, day programs, school, appointments, and errands. Per-trip or per-mile rates.
Help with bathing, dressing, grooming, meals, light housekeeping, and daily living support in clients' homes.
Help clients build independence — problem-solving, self-help and adaptive skills, plus coordination of counseling and support services.
Conduct Functional Behavioral Assessments and build plans that help clients change behaviors that may be harmful to themselves or others.
Lead activities that build community connections, life skills, and independence for adults on IO, Level One, and SELF waivers.
Help clients find, learn, and grow in community jobs — task training, assistive technology, and on-the-job coaching.
